Product Trial Equipment Agreement

TERMS & CONDITIONS – PRIOR TO SHIPMENT

1. Complete the Demonstration Equipment Agreement
2. Credit Approval is required
3. Credit Card Authorization (Purchase Order Number – Optional) is required for the following charges:
   1. Purchase of Fluids (non-returnable and non-refundable)
   2. Freight Charges

TERMS & CONDITIONS – DURING DEMO PERIOD

4. The Customer may use the Equipment for a period of five (5) days from the date Equipment is received

TERMS & CONDITIONS – AFTER DEMO PERIOD

5. The Customer shall return the Equipment to EBS-USA upon the expiration of the five (5) day demo period
6. If the Equipment is not received by EBS-USA within five (5) days of the expiration of the demo period, the Customer will incur a
   $100 per day charge, and the total amount will be charged to the credit card on file
7. Equipment returned to EBS-USA must be in the same working and physical condition as it was at the time the Equipment was
   delivered by EBS-USA to the carrier. To avoid additional charges, the following criteria must be met:
   1. Original packaging, if provided, must be used both inside and outside.
   2. All returns must be complete with all software, manuals, cables, warranty cards, power supplies, accessories, etc.
   3. Product must be clean and without scratches and marks.
   4. If the Equipment is not returned in such condition, in addition to any other amounts owing to EBS-USA, the Customer will be
      liable to EBS-USA for the amount necessary to restore the Equipment to such condition.
